In order to shape primary health care in the Tri-Cities, a variety of experiences and voices at the table are needed. The Tri-Cities Primary Health Care Network brings together community stakeholders to create partnerships and identify opportunities to improve how community members can access good quality health care in their community, across all ages and needs. They are looking for 2 patient partners to join them in their efforts by providing feedback on Primary Care Network portfolios.

Aim

Patients partners will:
  • Provide input in planning and implementation so that the outcomes lead to improved patient care.
  • Participate as a decision maker in how primary care services are designed and delivered in the Tri-Cities.
  • Use their experience to inform services so that they are relevant, accessible, meaningful and centred around patients, families and caregivers.

Level of Engagement

This opportunity is at the level of involve on the spectrum of engagement. The promise to you is that the health care partner will involve patients in planning and design phases to ensure ideas or concerns are considered and reflected in alternatives and recommendations.

Eligibility

Open to patient partners in the Tri-Cities-New West and surrounding communities who: • Ideally, have knowledge and experience in community health care, chronic disease management, wellness and community partnerships. • Experience sitting on a steering committee would be helpful, but not necessary. If you have a strong interest in this work but have not yet completed a PVN orientation and volunteer agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact the engagement leader directly.

Logistics

  • Commitment: There may be a minimal 1 week notice when attending any Primary Care Network meetings or phone calls. Dates and times are specific to each portfolio and are determined on a monthly bases and are subject to change.
  • Location: Third Wednesday of every month at Royal Columbian Hospital - 4:30-6:00
  • Date/time:  A combination of In-Person meetings and teleconferences up to one year, with an discussion of renewal.

Reimbursement

Preapproved expenses, per Fraser Health Travel Policy, related to local travel to in person meetings will be reimbursed including mileage, parking and/or public transit.

Background

Primary Health Care is meeting basic needs in the community for all users.  This engagement supports a newly developed steering committee consisting of three major partners: Tri-Cities Health Services (coordinators, managers, directors, executive director), Kwikwetlem First Nations and Fraser Northwest Division of Family Practice ( board executives, family physician reps, consultants). The Primary Care Network is being developed which is redefining how primary care in the community is delivered.
