Lead Organization/Department

Strategic Transformation, Primary and Community Care – Fraser Health

Aim

• To advance the practice, strategy and evaluation of team-based care in Fraser Health
• Patient advisors will share their care experience and be informed of Fraser Health work to provide advice on working group activities

Level of Engagement

This opportunity is at the level of consult on the spectrum of engagement. The promise to you is that the health care partner will listen to and acknowledge your ideas and concerns, and provide feedback on how your input affected the decision.

Eligibility

Patient Advisors from Fraser Health Communities who are interested in learning about Fraser Health, building new skills and

• Can speak to a lived experience with primary and community care
• Have experience with the elderly that have complex health issues and have used health care
services (One patient partner is requested with this background)
• We welcome a retired healthcare professional IE. a person who has knowledge of the health system

Please note: RSVP follow up will include an informal conversation about the opportunity and determination of the best fit for both patient and health care partners prior to placement

Logistics

• Vacancies: 2
• Date/Time: To be determined with patient partners
• Location: Virtual – Via Teleconference and/or Skype for business video conferencing
• Commitment: One-year term (with an option for renewal), monthly 90-minute virtual meetings in the afternoon

Reimbursement

No anticipated reimbursements currently. However, if the there is a change to in person, preapproved expenses related to travel to in person meetings will be reimbursed according to the Fraser Health Travel policy (including mileage each way, parking and public transit).

Background

Team-based care is a key priority in BC. It is a person- and family-centred approach to care that supports the provision of comprehensive shared care through a strong, interdisciplinary team of health care providers. The Team Based Care Strategy working group consists of a large group of Fraser Health Executive leaders (Directors and Executive Directors) that are focused on the successful transition to team-based care in FH.
