The Specialized Community Services Program Steering Committee is looking for up to three patient partners who would be interested in sitting on this committee to share their perspective. This work spans across complex medical and frail adults and adults living with mental health and substance use conditions.

The SCSP Steering Committee provides a forum for cross program and operational health care leaders to collaborate, share information and make recommendations to support the organizational planning and development of Specialized Community Service Programs for CMF and MHSU adult populations across the Central Interior Rural area. The Ministry of Health is prioritizing moving toward a more person-centered approach to care that will better address the health needs of the population. The scope of this work is vast and requires input, collaboration and partnership with a multitude of stakeholders. Patient partners will play a key role in the work and will participate monthly to review information, working group material and other related documents to the planning and decision making process.Level of Engagement

- Experience working in large groups
- Comfort in telling their story
- Familiarity in policy and procedure as this relates to health care
- Ability to read and understand health care material sent electronically in order to contribute to meaningful dialogue at the committee meeting

Specialized Community Services Programs (SCSPs) are designed by the Health Authority to provide continuity, flexibility and rapid mobilization of a set of responsive services to meet the needs of a defined population of people. SCSPs will provide personalized relationships between providers and patients with their families and caregivers and it may have one or more teams, each with linkages to the health system and broader community. SCSPs are also intended to incorporate a range of health authority services that were once separate structures and incorporate them into a single program. Many other stakeholders will be part of this work including patients, families and caregivers, Health Authority staff, physicians, NP, First Nation partners, non government organizations and many others.Health Care Partner Contact Information
