The Shared Care Committee (SCC) would like to work with patient partners to enhance an evaluation strategy intended to measure the impact of its initiatives. By providing structure for family and specialist physicians to work together, SCC initiatives strive to improve the flow of patients between primary and specialist care in BC. The purpose of the Shared Care Committee (SCC) is to support family and specialist physicians in working together to improve the flow of patients between primary and specialist care in BC. The Shared Care Evaluation Working Group (EWG) will provide advice and direction to the SCC on evaluation frameworks and deliverables that will enhance the SCC's ability to measure the impact of its initiatives on clinical improvement.Level of Engagement

- Experience with project management and/or evaluation is a great asset.

- Format: Monthly 60-minute teleconference meetings and quarterly 3-hour in-person meetings at the Vancouver office.

This is a new working group in early stages of drafting Terms of Reference which will be available to selected patient partners. The Shared Care Committee works collaboratively with patient partners on improvement initiatives across the province of BC. Overview: The relationship between the family and specialist physician is fundamental to the delivery of effective health care, especially for more complex patient populations. A growing number of sub-specialties along with patient management by physician teams in hospitals has contributed to care that is comprehensive but can be disjointed. Gaps in communication between hospital-based and community-based physicians, and between family and specialist physicians, can impede the flow of patients from primary to specialist care, especially for patients with chronic health conditions. Today many family and specialist physicians in British Columbia are working together through Shared Care initiatives to improve flow and bridge gaps in care to improve health and patient/provider experience. By facilitating collaboration between family and specialist physicians, Shared Care initiatives foster mutual trust, respect and knowledge of each physician’s expertise, skills, and responsibilities, all of which are integral to effective collaboration and collegian relationships. Long term, Shared Care work helps build a collaborative infrastructure for sustainable improvements for our health care system in BC.Health Care Partner Contact Information
