Background

Primary and Community Care, the program leading the PCQC, is a system transformation initiative, aimed at implementing a sustainable, patient-centred care model that has a foundation in primary health care services. The desire is to find means to “wrap services around” patients, especially people who have complex health needs, so that people experience seamless care, and that problems do not arise at transition and referral points in care provision. We know we can only achieve this systems transformation through collaborating with all parts of the health system. This involves bringing in patients’ voices in different ways, and working with patient volunteers, as well as agencies involved in health services, including Island Health, family physicians, community providers, and First Nations partners. Together, we are finding new ways to provide patients with the best health care services, at the right time and the right place, and to support patients in managing their own care.

PVN has really helped us engage with patient partners to improve health services at Northern Health.  It is more and more becoming the norm to include patients in the design, delivery and evaluation of health services.  PVN education and supports, involving patient partners, have enabled us to develop the capacity to include the patient voice to make care better and achieve better health outcomes.
