According to BC Renal Agency, the number of British Columbians with kidney disease is growing every year, due to our aging population and increasing rates of heart disease and diabetes. Fraser Health has three kidney care centres whose primary goal is to work with patients and families to preserve kidney function with the hopes of reducing the need for dialysis, and preparing for dialysis where needed. Your help is needed make these services more patient- and family-centred!

Aim

Patient partners will provide feedback as part of a planning team focused on improving kidney care services in Fraser Health to be more patient- and family-centred.

Level of Engagement

This opportunity is at the level of involve on the spectrum of engagement. The promise to you is that the health care partner will involve patients in planning and design phases to ensure ideas or concerns are considered and reflected in alternatives and recommendations.

Eligibility

  • Volunteers from Fraser Health Communities who are able to travel to Surrey for meetings.
  • This engagement is ideal for Patient Partners who have experience in chronic kidney disease or other chronic health challenges (such as diabetes or cardiac conditions).
If you have a strong interest in this work but have not yet completed a PVN orientation and Volunteer Agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact the engagement leader directly.

Logistics

  • Vacancies: 2-3
  • Date/time & commitment: 2-3 hour meetings, every 1-2 months for 7 months to a year. To be determined in consultation with patient partners.
  • Location: Most meetings will be at the Surrey Kidney Care Centre at 9801 King George Boulevard. This is directly across the street from the King George Skytrain Station. Some meetings may be held in Abbotsford and New Westminster.
  • Initial meetings are in person and then subsequent meetings via teleconference will be available

Reimbursement

Preapproved expenses related to travel to in person meetings will be reimbursed according to the Fraser Health Travel policy (including mileage, parking, tolls and/or public transit).

Background

Rationale: The last formal program review was over ten years ago. Recently, the team has noted many issues with the current workflow that impacts the patients’ experience, including longer appointment times. Team members: The interdisciplinary team includes clerical staff, nurses, dietitians, social workers and physicians.
