Getting the message out about what home support services are available is so important for families as they seek to access services for their loved ones. In today’s world, people access information through a variety of ways, whether it’s the internet or television. The Fraser Network Alliance recognizes an opportunity to inform and engage the public and are seeking patient partners, with experience in home support, to be a part of the development and production of videos that tell their story.

Aim

  • To partner with patient partners impacted by the use of home support services and share their story in the development and production of a video
  • Patient partner feedback will be incorporated into the vision and messages that is decided

Level of Engagement

This opportunity is at the level of collaborate on the spectrum of engagement. The promise to you is that the research partner will work together with you to formulate solutions and incorporate your advice and recommendations into the decisions to the maximum extent possible.

Eligibility

Patient partners who are
  • a senior being cared for in their home and/or their caregiver;
  • fluent in English;
  • have an interesting story to tell related to home supports, and are comfortable on camera
If you have a strong interest in this work but have not yet completed a PVN orientation and Volunteer Agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact the engagement leader directly.

Logistics

  • Vacancies: 2
  • Date and time: Meetings will be held monthly (to be arranged) and will be a mixture of in-person and telephone calls.
  • Location: For in-person meetings, Fraser Health Corporate Office, 10153 King George Blvd, Surrey, BC V3T 2W1
  • Commitment: Approximately 2 hours per month until late March. Please note that hours will most likely increase during the filming time, January 2018.

Reimbursement

Preapproved expenses related to travel to in person meetings will be reimbursed according to the Fraser Health Travel policy (including mileage, parking, tolls and/or public transit). *Please note: Free parking is available for 2 hours at Central City

Background

There has been a great deal of changes in Fraser Health related to Residential Care Optimization. This video is one way to communicate and inform the communities about these improvements. Members of the Fraser Network Alliance (that includes hospital, residential care, assisted living and home health representation) and the Aging Well in the Community: Residential Care Optimization Plan Team are brainstorming ways to increase awareness of some support resources available to seniors and their caregivers. The objectives are to:
  • elevate care giving in Fraser communities
  • identify the supports available during different stages of a senior’s journey.
Overall, this video aims to improve seniors’ quality of life, support their caregivers and help seniors stay home and healthy for as long as possible. The team is discussing creating a video to:
  • demonstrate services that promote independence
  • strengthen community connections,
  • facilitate access to community resources, and
  • promote timely access to health care services, for example, resources such as community day programs, in home support, and respite.
In addition to members of the Residential Care Optimization Team and the Residential Care Network Alliance, the working group aims to collaborate with the Fraser Health Patient Experience Department, the Patient Voices Network Engagement Lead, Home Health, a member of the BC Care Givers Association and a patient advisor.
